Positive findings in the Thomas Test include:

• If the thigh is held above the level of the table: psoas tightness (Figure 3.45(a))

• If the thigh abducts and the tibia is in external rotation: tensor fasciae latae and iliotibial band tightness (Figure 3.45(b))

• If the angle at the knee is greater than 90 degrees: rectus femoris tightness (Figure 3.45(c))

Myofascial tightness of the anterior thigh, however, can create a false positive for psoas tightness in this test. True psoas involvement is when the thigh is above the height of the table and you are able to palpate hypertonicity (tautness) in the psoas muscle belly, just inside the inguinal ligament. If the thigh is above the height of the table and no palpable hypertonicity is felt in the psoas, the leg is being held up by myofascial tension in the anterior thigh rather than by a short psoas (Osar 2014).
